Spiritual Growth Worksheet
At Crossroads we realize that every person has a different background and life experience. Because of that we want to offer individual spiritual assistance that will help you on your journey of faith. If you would be interested in meeting with our Pastor, he would love to get to know you, understand your unique spiritual journey, and assist you in your spiritual growth by developing a spiritual plan just for you. After a careful assessment of your Spiritual Growth Worksheet he will offer suggested steps that will help you to move forward in your journey of faith.
The Process
- Print out the "Spiritual Growth Worksheet" or fill out the online version of the form.
- Fill out the Work Sheet (see below)
- Mail the completed worksheet to: Michael V. Mallick @ office address or give it to Pastor Michael at church on Sunday or submit the online version directly on the web (see below)
- Make an appointment with Michael (church phone 920-3163…Allow for about an hour)
- Michael will pray, study the worksheet and notes from the one on one meeting, and come up with a list of suggestions to help you move forward in your walk with Christ.
- Preferably there is a second one on one meeting to discuss the suggestions. However, if the person cannot attend the second meeting the suggested plan can simply be mailed.

These questions are to help Pastor Michael get a sense of who you are and where you are spiritually. They will also help him to offer you the best suggestions to further your spiritual journey. Any questions that make you uncomfortable feel free to skip.
